Welcome to the application portal for the SSG Fox Suicide Prevention Program. We provide case management, peer support and referrals for eligible applicants that exhibit suicidal risk. Please respond to the following questions to the best of your ability, and a Regional Service Coordinator will reach out to you to complete the screening process within 24 hours (during normal business hours).
